Social Worker - Preparation for Adulthood Team

This is an exciting opportunity for a L2 Social Worker that has an interest in developing their skills supporting young people with disabilities and their families to prepare for adulthood.

The opportunity is within the preparation for adulthood team which sits in children's services within the birth to settled adulthood service. Birth to Settled Adulthood Service sees progress in first year

Your role will primarily be to complete preparation for adulthood plans with young people and their families/ carers. A preparation for adulthood plan focuses on how a young person is developing their independence. You will be offering information advice and guidance to support this and inform the plan.  

The plans are completed with young people aged 14 onwards and offer an opportunity to have conversations with young people and their families to understand what their needs might be in adulthood and how they can be supported to be as independent as possible.

You may also be required to complete an adult Care Act assessment and other adult social care activities, including undertaking Mental Capacity Assessments/Best Interests decisions.

We would welcome expressions of interest from practitioners who are working in either children's or adult services. This is a unique opportunity to develop skills and experiences across directorates.

You will work alongside colleagues with adult and children's social care expertise as well as education and Special Educational needs colleagues

This will be a fixed term post starting on the first day of employment within the team.

You must: 

• have a degree in social work or equivalent 
• be registered with the Health and Care Professions Council (HCPC) 
• be able to fulfil the travel requirements of the post
