About the role

We are looking for a motivated and skilled Social Worker to join our Home First North team, supporting timely and effective hospital discharges across the north of the county.

This is a rewarding opportunity to work at the heart of hospital discharge pathways, helping people return home safely and regain independence. You will work predominantly across Yeovil and Salisbury Hospitals, alongside a supportive multidisciplinary team and strong community links.

What you will be doing:

  • Supporting safe, person-centred hospital discharges via the P1, P2 and P3 pathways. 
  • Completing strengths-based assessments and care planning
  • Working collaboratively with NHS teams and community providers
  • Promoting independence through Home First principles
  • Taking part in duty arrangements as part of the team’s shared workload
  • Engaging in a weekly team day, providing connection, peer support, and shared learning

What we are looking for:

  • Qualified Social Worker (registered with Social Work England)
  • Experience of adult social care and/or hospital discharge work
  • Strong communication and partnership working skills
  • Ability to make timely, sound decisions in a fast-paced environment
  • Commitment to person-centred, strengths-based practice

Salary includes three additional labour market increments (LMI's) which are awarded on a temporary basis and are subject to regular (usually bi- annual) review in accordance with our labour market adjustment scheme.
