We have two exciting opportunities available for enthusiastic, committed and creative people to join the Adult Access Team as Assessment and Support Co-ordinators. Both are part time roles of 22 hours a week.
The Adult Access Team is the 'front door' to new and returning adult social care customers in the Dorset Council area and is a specialist adult social care team within customer services based at County Hall, Dorchester.
Using a strengths-based approach, we work with our customers to understand the outcomes they are looking to achieve, their strengths and assets and what support they may need to achieve their goals.
We aim to resolve as many enquiries as we can at the first point of contact by linking people with their local communities , providing information and advice, issuing small pieces of equipment which promote independence, considering what assistive technology may be helpful or whether a short term intervention is required to rebalance a person and support them to regain independence. If further assessment is required, we initiate referrals to our colleagues in the locality and specialist teams.

The ideal candidate will have some knowledge of adult social care, be a team player, have a willingness to learn, excellent communication skills and a passion for working with people, promoting their independence and helping them to achieve positive outcomes.

- The Adult Access Team is currently part of the Our Future Council transformation programme, phase 1. The Adult Access Team has provided a specialist adults function within customer services for many years and it is proposed that the team will ‘slot’ into the new Customer Hub with a new reablement function aligned to the team in the Autumn of 2025.
